After initial consultation, NP Services at Home's Board Certified Nurse Practitioner visits regularly with patients, not only monitoring chronic conditions, but also truly getting to know our patients. Timely medical assessment and intervention from NP Services at Home may prevent hospitalization and an extended skilled nursing facility admission. NP Services at Home is determined to minimize these episodes in order to maximize quality of life for our patients and their families.
